Financial Policy
We are committed to providing you with the best care possible to achieve total oral health. To achieve these goals, we need your assistance and your understanding of our financial guidelines.
Insurance
We accept all major dental insurance plans; however, we may not be a contracted provider for your plan. Our team would be happy to complete a complimentary benefit review on your behalf. In many cases insurance reimbursement is very similar.
Your insurance is a contract between you, your employer, and the insurance company. Please remember that our relationship is with you. Insurance is meant to reimburse the dentist for services retained by you, the patient.
- We are contracted with Delta Dental Premier and Aetna Extend
- No estimate is a guarantee of payment. You are responsible for all charges not paid by your insurance. Many insurance companies exclude certain dental procedures or downgrade procedures to a lesser reimbursement level; in these cases, you are responsible for the difference.
- Some specialized services, including but not limited to cosmetic procedures, orthodontic procedures, and porcelain restorations, incur additional fees associated with the fabrication of such items. These fees are not billable to dental insurance and are the responsibility of the patient. All fees will be presented ahead of time if applicable
Payments
- For appointments scheduled for over 70 minutes in length, a $100 deposit is required. This deposit will be applied toward your co-pay if the appointment is attended. If the appointment is missed or rescheduled outside of a two business days’ notice, the deposit may be used as a cancellation fee.
Cancellation Fees
- Hygiene appointment cancellations made within two business days’ notice are subject to a $50 fee.
- Doctor-scheduled treatment appointments cancelled within two business days’ notice are subject to a $100 fee.
Payment Information
- All major credit cards are accepted (Visa, MasterCard, Discover, American Express).
- Various payment options are available to all patients, including financing options with CareCredit® and iCreditWorks. Speak with your Treatment Coordinator for more details.
- A 3% convenience fee is applied to credit card transactions. HSA cards, debit cards, CareCredit, cash, and checks are exempt from this fee.
- Balances left over 60 days will incur a $25 minimum late payment charge. We realize that temporary financial problems may affect timely payment of your account balance. If such problems arise, we encourage you to contact us promptly for assistance in managing your account.
- Minors must be accompanied by a parent or legal guardian. If the parents are separated or divorced, the person accompanying the minor will be responsible for copayment at the time of service.
Appointment Policy
Our office reserves chair time for every scheduled patient. In respect for all our patients, we kindly request two business days’ advance notice to change or cancel an appointment.
- We reserve the right to charge a fee for missed appointments and/or appointments cancelled or rescheduled without adequate notice (see cancellation fees above).
- This visit fee is due at the time of the appointment.
- If rescheduling is appropriate, the visit fee may be applied as a credit toward the rescheduled examination within 30 days.
